Appointment Request

If you are new to our office you will need to complete the appointment request form to get started. We will do our best to respond within 2 business days.

At this time, our licensed therapists have limited availability. If you want to be seen quickly, you may register to meet with one of our Associate Therapists below.

If you plan to use insurance, please have your insurance card ready so you may upload a picture of it. This will help our scheduler verify your benefits prior to scheduling. Once your form is submitted our scheduler will review your information and contact you regarding therapist availability.

Please make sure to review the FAQ page for information on insurance, fees, etc.

Important info on:

  • Insurance does not consider counseling for communication issues medically necessary and therefore will not cover such services. Counseling for couples and families, which often addresses communication, relationship dynamics, and conflict resolution, is viewed as a personal issue rather than a medical one. These appointments will be charged at the standard rate.

  • For children of divorced parents or those with legal guardians, we need proof of the consenting adult's legal authority to approve treatment before services begin. This includes divorce decrees, custody agreements, or guardianship papers.

  • Sessions are at a reduced, self-pay rate ranging from $65-$75/session. Services are not billable to insurance.

  • Tuesday-Thursday from 4PM-8PM, every other Saturday 9AM-1PM (Online)

  • Children (ages 8+), Teens, and Adults (not couples)

  • Sessions are at our standard rate ranging from $105-145/session. Services are billable to insurance.
